Participation Agreement 2019

  • Thank you for your interest in becoming a Directing Change school!

    By submitting this form you are agreeing to the following responsibilities:
    1. Review of all contest rules and guidelines
    2. Sharing at least one of the educational resources (lesson plan, Prezi, short videos) from the Directing Change website
    3. At least one film submission to the film contest (deadline March 1, 2019)
    4. Sharing student produced film(s) with others on campus


    Here are some of the benefits of being a Directing Change school:
    1. Students learn the warning signs of suicide and are prepared to reach out for help for themselves or a friend.
    2. Listed on Program Website
    3. Preferred access to AB 2246 trainings, technical assistance, and support.
    4. Ability to request Directing Change Team members to present to parents, students, and classrooms
    5. Support with implementation of What I Wish My Parents Knew events and other mental health and suicide prevention events.
    6. Notification of special mini grant opportunities
  • Generally this is the teacher, counselor, etc. in charge of the group
  • I agree to participate in the Directing Change Program for the 2018-2019 school year. The teacher/adult advisor listed above will implement the program within the classroom, after school program, club, or other group listed above.

Directing Change is part of statewide efforts to prevent suicide, reduce stigma and discrimination related to mental illness, and to promote the mental health and wellness of students. These initiatives are funded by counties through the Mental Health Services Act (Prop 63) and administered by the California Mental Health Services Authority (CalMHSA), an organization of county governments working to improve mental health outcomes for individuals, families and communities.
